Experiences of MEBO Dressing for Accelerating Wound Healing in Treating Whitlow and Paronychia

¡¾Abstract¡¿Objective£ºTo observe the effect of MEBO Wound and Ulcer Dressing on bacteria infection control and regeneration of peripheral nerve and peripheral tissue. Method£º50 cases of patients with whitlow and paronychia were treated with MEBO Wound and Ulcer Dressing without anti-infection and decompression. Results£ºAmong of all cases£¬42 cases were cured£¬3 cases gave up the treatment and 5 cases did not follow up. No dysfunction and discomfort in all cases were observed. Conclusion£ºMEBO Dressing for Accelerating Healing has significant effect in treating whitlow and paronnychia£¬and can obviously relieve local pain caused by nerve stimulation by inflammatory substance.

Paronychia, called as ¡®snake-head sore¡¯ in Chinese traditional medicine, has specific clinical manifestation. From Feb, 2005 to Mar, 2009, our department used MEBO Wound and Ulcer dressing in the treatment of paronychia, gained good therapeutic effect. And the report as followed:

1. Clinical material
    There were totally 50 patients, 38 males, 12 females. The age is between 2 to 45 years old, with the average age 21.7. All patients were diagnosed as purulent whitlow or infectious paronychia, the affected areas locate at the top or bilateral sides of forefinger, little finger and ring finger with pressing signs and symptoms in different degrees, such as the apparent pain, the quality of pain is pulsating pain and tenderness in dependent position that severely affect the work and rest. Among them, there were 18 cases with the infection affecting bilateral nail groove, 7 cases with infection at nail groove and nail bed, 12 cases with purulent infection on the top finger and pressing symptoms and 13 cases with mixed infection that means the combination of paronychia, infection of nail beds and purulent whitlow).
2. Therapeutic procedures
2.1. Restrict the movement of affected limb in order to reduce the venous stasis and decrease the pressing symptoms.
2.2 Provide bland and digestible food. Restrict the consumption of spicy, irritating and high-carbohydrate foods, such as cigarette, alcoholic drinking, seafood and etc.
2.3. At the initial stage of treatment, MEBO Wound and Ulcer Dressing was topically applied, encircling the entire finger and put it under the irradiation of  TDP lamp for 10 to 30 minutes. According to the patient¡¯s compliance, coordinate the distance between the lamp and the affected finger. It is prohibited to put too close to cause local burn injuries; if it had formed the ulcer with the purulent discharge at the local sites, 0.9% NS should be used to repeatedly rinse the wound till complete removal of purulent or bloody excreta. Then, we applied MEBO Wound and Ulcer Dressing on the affected area exceeding and fully covering the wound bed with proper pressure. Changed the dressing 2 times per day (the previous 3 days), then changed to 1 time per day. When the purulent-like discharge was seen on the wound bed during the dressing-change, the cotton swab impregnated with normal saline should be used to gently wipe the wound bed and surrounding area. And then keep on using MEBO Wound and Ulcer Dressing, till the cessation of pain, swelling and exudation.  
3. Result
Among of all cases£¬42 cases were cured£¬3 cases gave up the treatment and 5 cases did not follow up. No dysfunction and discomfort in all cases were observed.
4. Discussion
Purulent whitlow and paronychia are the common infectious disorders. The occurrence of these two disorders will severely affect the daily work and life. In severe situation, they can cause the local bone infection, even the osteomyelitis and the muscular and skin necrosis around the finger top. For severe patient, it will cause sepsis and bacteremia and infectious phlebitis and etc. But the 50 patients in this report had not the occurrence of the above complications. It is believed in traditional Chinese medicine, the mechanism of surgical disorders is the result of heat and blood stasis or the invasion of external factors. The therapeutic principles are major in removing the necrotic tissue and promoting the growth of muscular tissue, clearing the heat and detoxication, promoting blood circulation and removing blood stasis. Prof. Xu proposed in the researching process of Burn Regenerative Medicine and Therapy that remove the necrotic tissue by the way of liquefaction without damage¡¯ and ¡®Regeneratively restore the wound bed via the in situ transplantation of stem cell¡¯ that is another whole new science in the modern medicine. MEBO Woudn and Ulcer Dressing effectively protects the wound bed, relieve the local symptoms (such as redness, swelling, heat, pain and etc) and maintain the wound bed in the physiological moist environment to prevent the nerve endings from the direct irritation and injuries by air and inflammatory tissues. Then, the sensitivity of nerve endings will be decreased and the rapid water evaporation can effectively be relieved to improve the stasis of local circulation and prevent the cellular death from dehydration. Berberine, one of pharmaceutical components contained in MEBO Wound and Ulcer Dressing has the functions of anti-inflammation, removal of micro-circulatory dysfunction and the relaxation of the spasm of piloerection. Then, the good analgesic effect and the decreased absorption of inflammatory substances will be attained.   
Aside from the protection, MEBO Wound and Ulcer Dressing can also decrease the chance of infection induced by environmental factors. In addition, ¦Â-sitosterol---the medical component in it has the anti-inflammatory and anti-bacteria effects. And the baicalin contained in it has the effects of anti-epinephrine and anti-catecholamine. Hence, MEBO Wound and Ulcer Dressing can exert the functions that include the relief of vasospasm, anti-oxidation, resistance against the stress response and the modulation of immunity. 
Due to large amounts of multiple amino acids, fatty acids and saccharides contained in MEBO Wound and Ulcer Dressing, it has the effects of nutrition to the metabolism of tissue cells and protection to the cells. Besides, it can increase the tension of cellular membrane. And also can it regenerate and repair the cells that are destructed by inflammatory tissue. As a result of the maintenance of moist environment on the wound bed functioned by MEBO Wound and Ulcer Dressing, it is helpful to regenerate and repair the tissue structure, and then decrease the scar formation after the healing of the wound. 
It is convenient to use MEBO Wound and Ulcer Dressing with wide-range of application. In addition to the patients with whitlow and paronychia who are given this product and technique, we also use it on the patients with traumatic hematoma that the bandage with pressure on the wound bed can promote the absorption of hematoma. Besides, it is used in the wound treatment of varicella. The therapeutic results are incredible, such as no scar formation, the relief of the severe pain led by the neuritis that is the result of the concurrent infection during incrustation stage of herpes zoster. In conclusion, the good therapeutic effects were gained in the treatment of the affected sites with incrustation or crust desquamation.
